Please arrive 10-15 minutes early for your Appointment for registration and to fill in a health questionnaire.

To better serve your needs and facilitate your consultation with our spine physician, please bring the following items to your visit:

• A list of current medications
• Information on treatments and medications tried in the past
• Any and all possible films (imaging studies) done at other facilities
• Medical records from prior visits
• Insurance information and cards

To cancel an Appointment

Telephone the office during business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
